For Confidentiality and HIPAA reasons we will now need to know who will be picking up any information you request to be left at the front desk for pick up. To make sure that information in regard to your child does not get to the wrong person, the employee that talks to you will ask who will be picking up the item you need and will notate that on the front of the envelope being picked up. Then when you pick the info/ envelope up from the front desk, they will verify that you are the person listed on the front of the envelope. If we do not already have a scanned photo I.D. of that person in our computer system – they will ask for a form of photo I.D. before they can give you the envelope.
We apologize for any inconvenience this may cause but the confidentiality of your child’s information is of extreme importance to us.
